{"id":69,"date":"2013-07-15T10:39:18","date_gmt":"2013-07-15T09:39:18","guid":{"rendered":"http:\/\/www.vision-laser.fr\/?page_id=69"},"modified":"2020-12-15T11:42:05","modified_gmt":"2020-12-15T10:42:05","slug":"traitement-medico-chirurgical-du-glaucome","status":"publish","type":"page","link":"http:\/\/www.vision-laser.fr\/en\/le-glaucome\/traitement-medico-chirurgical-du-glaucome\/","title":{"rendered":"Glaucoma treatment: the medical-surgical process"},"content":{"rendered":"<div>\n<p class=\"translation-block\">Glaucoma treatment aims to lower eye pressure to reach a <strong>target pressure<\/strong>.<br>\nTarget pressure is the pressure under treatment that prevents progression or the appearance of visual field deficits.<br>\nA 20% reduction in initial pressure is considered sufficient in most cases.<br>\nA 25% reduction in pressure reduces the risk of progression by 45%.<br>\nIn the case of advanced glaucoma, the pressure must be less than 18 mm Hg and in some cases lower.<br>\nAny diagnosed glaucoma must be treated either with eye drops, or with <a href=\"http:\/\/www.vision-laser.fr\/en\/le-glaucome\/laser-slt\/\" target=\"_self\">Laser treatment<\/a>, or with surgery.<\/p>\n<h2>Medical treatment<\/h2>\n<p>Almost always in the form of eye drops which must be instilled regularly for better effectiveness.<\/p>\n<p>The choice of eye drops prescribed as first-line treatment depends on its tolerance, its efficacy and its contraindications and adverse effects.<\/p>\n<ul>\n<li>Certains agissent en baissant la s\u00e9cr\u00e9tion de l\u2019humeur aqueuse de l\u2019\u0153il :\n<ul>\n<li class=\"translation-block\">Collyres b\u00eata bloquants (Timoptol, Carteol, Betoptic\u2026)<br>\r\nMais il faudra au pr\u00e9alable s\u2019assurer de l\u2019absence de contre-indications (maladie cardiaque de la conduction, asthme, diab\u00e8te insulinod\u00e9pendant etc.\u2026).<br>\r\nIls sont le plus souvent prescrits en premi\u00e8re intention \u00e0 raison d\u2019une fois ou deux par jour.<\/li>\n<li>Certains collyres sont des inhibiteurs de l\u2019anhydrase carbonique dorzolamide (Trusopt, Azopt)<\/li>\n<li>Citons \u00e9galement les collyres alpha 2-agoniste (Alphagan, Lumigan &#8230;)<\/li>\n<\/ul>\n<\/li>\n<li>D\u2019autres collyres agissent au contraire en augmentant l\u2019\u00e9limination de l\u2019humeur aqueuse :\n<ul>\n<li>Collyres Adr\u00e9naline et d\u00e9riv\u00e9s (Epinephrine)<\/li>\n<li>Collyres Prostaglandine (Xalatan, Travatan) tr\u00e8s prescrits.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>These treatments can cause a change in eye color, eyelash changes and, like all other eye drops, eye irritation upon instillation or even allergies.<\/p>\n<p>Finally, let us add that each class can be combined with another in the same bottle in order to increase effectiveness:<\/p>\n<ul>\n<li>Beta blocker and pilocarpine (CARPILO, TIMPILO),<\/li>\n<li>Anhydrase inhibitor and adrenergic antagonist (COSOPT),<\/li>\n<li>Prostaglandin and beta-blocker (XALACOM).<\/li>\n<\/ul>\n<p>In practice, beta-blocker eye drops are often prescribed as a first-line treatment, but some people also prefer to use a prostaglandin because of its lower risk of side effects.<\/p>\n<h2>Glaucoma Treatment: Surgical Mode<\/h2>\n<p>Glaucoma treatment by surgery, although rarer, remains the last resort when medical or laser treatment has failed (increased pressure, worsening of the visual field, treatment not followed).<\/p>\n<p>When treating glaucoma by surgery, the operation is performed either on an outpatient basis or after a short hospitalization.<\/p>\n<p>It involves trabeculectomy or deep sclerectomy.<\/p>\n<p>Both techniques create an additional passageway for the evacuation of aqueous humor.<\/p>\n<p>Unlike cataract surgery, during which vision returns quickly, glaucoma surgery results in poor vision in the days that follow, which lasts for a few days.<\/p>\n<p>Furthermore, it does not repair damage to the optic nerve that has been permanently destroyed.<\/p>\n<p>In expert hands, the success rate of glaucoma surgery on previously unoperated eyes is approximately 90% at 2 years.<\/p>\n<\/div>","protected":false},"excerpt":{"rendered":"<p>Le traitement du glaucome vise \u00e0 faire baisser la tension oculaire pour atteindre une pression cible.
